Donation Forms

Please use the following documents when soliciting our local merchants for donations. The merchant Donation Letter should accompany the Donation Agreement form whenever soliciting a new merchant. The Donation Agreement Form is usually provided as a triplicate so you may need to make a copy for the merchant when submitting the form to the PTA.
